How someone can reclaim my passport for me?

In the case when you are unable to reclaim the travel document in person due to an illness, disability or other obstacle you are unable to overcome, reclaiming of your passport may be conducted by another person (attorney) holding a special power of attorney provided by you, drawn up before a notary for reclaiming the document and a document stating that you are unable to make the trip to reclaim the document in person.
The attorney may reclaimed your passport in the seat of the Head of the Office for Foreigners at 33 Taborowa Street in Warsaw, from Monday to Friday, between 09:30 – 14:30 following the previous submission of an application by you for its provision.
